There are two approaches in rhinoplasty surgeries: open and closed techniques. Although both techniques have been used for years, many surgeons still recommend open technique rhinoplasty to their patients.
The purpose of rhinoplasty is to eliminate the deformities in the nose in the most natural and beautiful way. What should not be forgotten is that everyone needs a different nose structure and therefore a special treatment plan. At this point, the technique to be used in surgery is of great importance.
The main difference between open technique rhinoplasty closed technique rhinoplasty is that open nose job provides a wide field of vision to the surgeon. In the open technique, a small surgical incision is made in the lower part of the nose, in the middle of the nostrils, and the entire nasal skin is removed with the help of this incision. Thus, all cartilage, concha, and bone structure can be observed easily and even the most severe deformities can be easily corrected by a specialist and experienced surgeon. On the other hand, the incision is made into the nose and the nostrils are entered directly in the closed technique. This method is preferred because both the operation time and the recovery time are short. Although some surgeons believe that postoperative bruising and swelling are not encountered as much as in the open technique, this has not been proven definitively in the literature. Such postoperative complications depend on the technique used, as well as the nasal skin of the patient. Edema and swelling can be seen for a longer time in patients with thick nasal skin. The biggest problem with this method is that it is not suitable for every case. While closed technique rhinoplasty is successfully performed only in the noses with minor deformities, open rhinoplasty is suitable for all patients regardless of the size of the deformation.
Since the surgical incision made in the closed technique remains in the nose, it is also known as scarless rhinoplasty. However, also in open rhinoplasty surgery, the incision made from the tip of the nose is very small and it is very difficult to notice once it’s fully healed. In this sense, it would not be reasonable to prefer a closed technique only to avoid surgical scars.
It is essential to use the open technique if a revision rhinoplasty procedure is required in patients who have had a previous surgical procedure on their nose.
With the closed technique, a proper bone structure and a successful result can be obtained in suitable cases. However, with the open technique, the chance of obtaining a healthy and beautiful result in every nose structure is always higher. Of course, some surgeons successfully perform both methods. There is no definite consensus on which approach is more advantageous in rhinoplasty surgery, and each surgeon chooses the technique he deems appropriate for his patient. Another factor that is as important as the technique used is the surgeon's dexterity and experience. When these elements complement each other, the result will be beautiful. If it is my personal preference, it has always been an open technique for the reasons I explained above.
